Fortitude Valley’s experienced nutritionist to get you on track

When is the right time to start a healthy relationship with yourself about your health? There is never a better time than NOW. We are very excited to introduce a fabulous new program to help you change your life for good. Our dietitian and experienced nutritionist Dorelle Thompson is a certified My health for Life facilitator.

Fortitude Valley group sessions

My Health for Life is a new 6-month lifestyle and behaviour modification program aimed at eligible participants who are likely to develop type 2 diabetes, heart disease and stroke. The program works with participants to identify their barriers to change and supports them to set realistic health goals and action plans.

About the program

  • The program is free
  • It is delivered by a qualified health professional in Fortitude Valley 
  • It involves six sessions over six months – the time commitment required is not big 
  • The first session is a personalised one-on-one appointment 
  • The remaining sessions are small groups coaching sessions. If you miss a session due to sickness or other reasons, there is also an option for catch up sessions to keep you on track

  • The program is also available via phone coaching.

Eligibility

  • People aged 45 years and over who have been identified by a health professional as being at high risk of chronic disease (which includes type 2 diabetes, heart disease and stroke)
  • People aged 18 years and over who are of Aboriginal or Torres Strait Islander descent who are at high risk of chronic disease
  • Age 18 years and over with pre-existing conditions (E.g. previous history of Gestational diabetes or have been diagnosed with high blood pressure or high cholesterol)
To determine if you are eligible you will need to complete a free ‘check your risk’ form at www.myhealthforlife.com.au or call 13 RISK (13 7475) where you can take a free health check over the phone. If you would like our dietitian to help you complete your risk assessment, please call 0421 933 459 to arrange a time.


If you are eligible to take part, the next step is to book in for the first individual session. 
Note: you will need to complete the individual session before the group's sessions start.
