Diabetes Group Lifestyle Program

Is this for you?
        People living with diabetes who want to prevent complications and  improve health

New Sessions start Tuesday 1 August

Program Includes:
  • One individual nutrition assessment with dietitian/nutritionist
  • Five group learning sessions - bi-weekly interactive learning sessions that are informative and fun
  • One supermarket tour
  • Resource folder with notes and tips to support you in making changes
  • The aim of each session is to provide a supportive environment for you to gain new skills, information and practical solutions for you to make the healthy choices the easy choices
Two optional follow-up sessions will be available for additional support:
  • Adjusting for change
  • Dealing with setbacks
  • Revising and adjusting plans
  • Engaging support and sharing your journey

Program Fees - Free
You are eligible to be bulk billed under the Medicare Enhanced Primary Care (EPC) Program if you are: 
  • A type 2 diabetic, and
  • You will need a GP Management Plan, and completed and signed Allied Health group services Referral Form, from your GP.
  • Note: group services are available in addition to the 5 individual EPC’s allied health services under Medicare
How to get started:
  1. Make an appointment with your GP to complete/review a GP Management Plan and an Allied Health Group Services Referral form (Diabgrp 0510). 
  2. Book in for a Diabetes Group Assessment (must be completed prior to starting group sessions) with the program coordinator Dorelle Thompson. Bring a copy of the referral from your GP to the assessment or have your GP fax the completed referral to the program coordinator on 07 3103 4619.

Group Size: 6-12 people (Note: sessions will not start with bookings of fewer than 6 people).

Session 2 – Diabetes and You(Dietitian/Nutritionist)

  •  Overview of program and sessions
  • Guidelines for success; expected outcomes
  • Overview of Type 2 diabetes - causes, complications  Lifestyle and risk factors
  • Behaviour change

Session 3 – Behaviour  Change   (Dietitian/Nutritionist)

  • Finding your motivation
  • Understanding your goals
  • Planning for success and failure
  • Adjusting for change

Session 4 – Food and Nutrition   (Dietitian/Nutritionist) 

  • Balancing your calorie and nutrient intake
  • Where  is the energy coming from?
  • Important nutrients for diabetes
  • Introduction to meal planning

Session 5 – Food and Nutrition
Dietitian/ Nutritionist

  • Weight concerns
  • Advanced meal planning
  • Label reading
  • Eating out

Session 6 – Shopping Smarter(Dietitian/Nutritionist)

  • Shopping strategies for supermarkets 
  • Marketing – are you buying foods you don’t need?
  • Review of program components
  • Feedback, challenges and evaluation
