One individual nutrition assessment with dietitian/nutritionist
Five group learning sessions - bi-weekly interactive learning sessions that are informative and fun
One supermarket tour
Resource folder with notes and tips to support you in making changes
The aim of each session is to provide a supportive environment for you to gain new skills, information and practical solutions for you to make the healthy choices the easy choices
Two optional follow-up sessions will be available for additional support:
Adjusting for change
Dealing with setbacks
Revising and adjusting plans
Engaging support and sharing your journey

Program Fees - Free
You are eligible to be bulk billed under the Medicare Enhanced Primary Care (EPC) Program if you are:
A type 2 diabetic, and
You will need a GP Management Plan, and completed and signed Allied Health group services Referral Form, from your GP.
group services are available in addition to the 5 individual EPC’s allied health services under Medicare
How to get started:
Make an appointment with your GP to complete/review a GP Management Plan and an Allied Health Group Services Referral form (Diabgrp 0510).
Book in for a Diabetes Group Assessment (must be completed prior to starting group sessions) with the program coordinator Dorelle Thompson. Bring a copy of the referral from your GP to the assessment or have your GP faxthe completed referral to the program coordinator on 07 3103 4619.

Group Size: 6-12 people (Note: sessions will not start with bookings of fewer than 6 people).
